Safety behaviour observations

Anyone on site can log a positive or at-risk observation from their phone in under 30 seconds. At-risk observations are routed for action automatically, and every one feeds a live picture of safety behaviour across your sites, inside your Microsoft 365.

What changes on site

No chasing paperwork.

More observations, not fewer.

No manual filing.

No obersavtions left hanging or hidden on a supervisors desk.

No lost records.

Every at-risk observation gets an owner, a due date and a closed-out record.

The problem

Observation cards go in a box. Nobody sees the pattern.

Low uptake

Too much effort to report

Cards run out, pens don’t work, and nobody wants to walk back to the cabin to fill one in.

Unactioned

At-risk stays at risk

Cards wait to be typed up, by which time the hazard has moved on, or the incident has happened.

Invisible

No picture of the trends

Without data by site, trade and category, toolbox talks and campaigns are aimed by guesswork.

Thankless

No feedback to the observer

People stop reporting when they never hear what happened, and good practice goes unrecognised.

Live demo

Follow an observation from spot to trend.

An at-risk observation at Northfield Construction’s Dartford Works site. Click any step, or let it play.

  1. Anyone on site scans the QR code, picks positive or at-risk, chooses a category and trade, and adds a note and a photo. It takes under 30 seconds, with no app to install.

  2. At-risk observations go straight to the right site manager or contractor supervisor, with a due date. Positive ones send a thank-you to the person's supervisor.

  3. The owner records what was done, adds a photo as evidence and closes it out. The observer is told it's been dealt with, so they keep reporting.

  4. Every observation feeds a live dashboard: positive to at-risk ratio, top categories, trades and sites, and any actions running late.

What it changes

  • More people reporting. Logging takes seconds, so observations become a habit.
  • Hazards closed faster. At-risk observations reach an owner straight away.
  • A positive safety culture. Good behaviour is recognised, not just bad behaviour flagged.
  • Targeted toolbox talks. Aim briefings at the trades and categories that need them.
  • Leading indicators for the board. Report on safety before incidents, not after.
  • Less admin. No typing up cards or rebuilding spreadsheets.

Questions

Common questions about digital observations.

Can people report anonymously?

Yes, if you want them to. Anonymous reporting can be switched on or off, and the site, trade and category are still captured so the data stays useful.

Do we have to use your categories?

No. We use your existing observation categories, trades and contractor list, and you can update them yourself later in SharePoint.

Who is told about an at-risk observation?

You decide. Observations are routed by site and category to the site manager, H&S advisor or contractor supervisor, with a due date for the action.

Can subcontractors use it?

Yes. Anyone with the QR code or link can log an observation from their own phone. They don't need a Microsoft 365 licence or an app.

What do we do with the data?

The dashboard shows trends by site, trade and category, plus your positive to at-risk ratio, so you can target toolbox talks and report leading indicators to the board.
